Medicaid Updates and New Medicaid Form

Important Updates about Medicaid

 

NKESC has received some clarification and updates in regards to Medicaid and Medicaid billing. Some of the information received has invalidated the directions given to staff at the MIS trainings. At the MIS trainings staff were directed to disregard the Medicaid billing form that has been traditionally used. This was an error. Staff must still get the initial Medicaid Billing form signed. Due to this new requirement, there will now be two Medicaid Billing forms. They are:

 

Form 1: “Medicaid Reimbursement Consent” is the form we have traditionally used. You are required to get this form signed once when a student is first placed on an IEP or moves in from another district. IMPORTANT: At the MIS trainings you were told that this form is no longer required. That is not true, this form continues to be a requirement for Medicaid billing and must be completed one time. The original signed copy is then sent to Kathy Wentz. This form has always been and will continue to be a PDF document only and is available at the bottom of the forms list in the Net IEP system.

 

Form 2: “Medicaid Health Authorization” is the new form. It is available in a PDF format on the NKESC website. It has also been attached to this document as a PDF. This form will eventually be in the forms list for every student on an IEP. We will notify everyone once it is online in the Net IEP system. This is the form that is used on an annual basis for obtaining a physicians consent in order to bill for specific services as specified in the IEP. As stated in the MIS trainings, this form must be completed when Speech, Occupational Therapy, Physical Therapy, Audiology, or Nursing are listed as service on the student’s IEP. This form needs to be completed and submitted initially by November 1st. Having the form signed does not require an IEP meeting. If you have an annual IEP before November 1st you can have it signed at the meeting. If your annual IEP due date falls after November 1st, you must have it signed before November 1st and then do it again at the annual IEP meeting.

 

NOTE:

A Physician’s name and address must be listed on the consent form. If you don’t know the address at least put the name of the town the physician works in. The name of a clinic or hospital does not suffice. (If the parents do not have a physician, leave the section blank and attach a note explaining this to Kathy Wentz.)

If the parents does not have the Medicaid card, you still need to complete the rest of the form and submit it to Kathy Wentz.

The original signed copy must be submitted to Kathy Wentz.

There were concerns raised about billing for two different services on the same day. NKESC has consulted with Medicaid and has been assured that if Kathy Wentz codes the services correctly, that billing for two services on the same day can occur.
